麻豆原创 uses a variety of forms that you may have to use at some point during your 麻豆原创 journey. You’ll access these forms through our online forms portal, Etrieve.
Directions for Logging into Etrieve
- Using Chrome, go to
- Log in to your account
- Click on “Forms,” then select specific form that you need from the menu
- Complete the form as prompted, including any attachments that may be required
- Click the “submit” button at the bottom of form
- Make sure to close your browser when you are finished
